Clinical Exposure Designed for International Medical Graduates
U.S. Clinical Experience (USCE) plays a critical role in strengthening an IMG’s profile for residency, fellowship, and other physician pathways. It’s worth noting that we don’t recommend this for everyone — in practice, USCE benefits a smaller subset of candidates than many assume. Nevertheless, everyone thinks that if they need to train or work in the USA, they need a USCE. This is not always the case. But if this applies to you, these experiences — often referred to as clerkships or externships or observerships or many other names — may help programs that you have spent some time in the U.S. healthcare system.
At IMG Secrets, we focus on quality, relevance, and individualized exposure, rather than volume-based rotations.
When U.S. Clinical Experience Matters
For many international medical graduates, strong academic credentials alone are not enough. Programs want to see:

Program Overview
Our U.S. Clinical Experience program is designed to provide focused, hands-on exposure in real clinical environments.
Wherever possible, we intentionally limit each rotation to one IMG per site, ensuring:
- Personalized supervision
- Better learning opportunities
- Stronger performance-based evaluation
This approach allows candidates to gain deeper clinical insight and meaningful mentorship — rather than being one of many observers.
What’s Included in the Program
The USCE program fee includes:
- Participation in a supervised U.S. Clinical Experience rotation
- Opportunity to attend patient consultations and gain hands-on exposure, as permitted by the clinical setting
- Visa support letter (if required) *The final decision for the visa is made by USCIS and not IMG Secrets LLC.
- Letter of Recommendation (LOR) for U.S. or Canadian residency applications, based on performance and evaluation
Each experience is structured to help you demonstrate professionalism, clinical understanding, and readiness for the next step in your career.
What’s Not Included
The following are not included in the USCE program fee and remain the responsibility of the candidate:
- Securing a visa from the U.S. Embassy
- Travel, accommodation, and local transportation
- Food and personal expenses
- Procedures or activities requiring additional liability coverage

The initial consult will include a strategic career guidance with situation analysis and guidance. In addition, the appointment fee is credited toward your U.S. Clinical Experience program fees if you proceed.
